The first step is a visual evaluation of your legs combined with a review of pertinent history. A screening Doppler is then used to evaluate common "leaky spots," in the front and back of the legs. "Leaky spots" known as reflux in phlebology terms, require that further evaluations be performed. The goal is to accurately determine the areas of the venous dysfunction so an accurate care plan can be made. After your evaluation, compression hose will be prescribed. The length and strength will be determined by the degree of the venous insufficiency identified. If your problem is superficial and the veins are small, compression sclerotherapy is the treatment of choice. In this procedure a fine needle is placed in the vein, the leg is elevated to empty the vein, and then the vein is injected with a sclerosing solution. A cotton ball or special pad is placed on the vein and the compression hose then holds this in place. Walking after this procedure is highly encouraged.

If "leaky spots", reflux, are found at the junctions between the superficial and deep system, treatment options include Ultrasound Guided Sclerotherapy or ligation.

The ultrasound can image a vein. Then a needle is accurately placed into the vein and a sclerosing solution is injected. In ligation, the area is numbed and an incision is made through the skin. The vein is located and then tied with a suture and cut. This effectively stops the abnormal flow of the blood.

Veins may be removed through hook phlebectomy. The skin is numbed and a small puncture is made. A vein hook is introduced and the vein is brought to the surface. The vein is then removed through a series of these small punctures.
